Fiorentina was defeated 2-1 by Jarrod Bowen's stunning injury-time goal, giving the Hammers their first trophy since the FA Cup in 1980.
After overcoming West Ham's 43-year trophy drought, David Moyes presented his father with the Europa Conference League trophy.
On a momentous and emotional night in Prague, Jarrod Bowen's spectacular injury-time strike defeated Fiorentina 2-1 and gave the Hammers their first title since the FA Cup in 1980 and their first European crown since 1965.
At the conclusion of West Ham's European tour, Moyes' father, 87-year-old David Snr., joined his son on the field.
"He was the first one, I thought I should get it around his neck," exclaimed the elated West Ham manager. Over the years, he has watched a lot, and I hope he loved it.
On the hour, West Ham took the lead thanks to a Said Benrahma penalty, but Giacomo Bonaventura's goal cut the lead short.
Bowen's pursuit of Lucas Paqueta's ball over the top, though, nearly led to Moyes adopting a Jose Mourinho-like demeanor.
He said, "I was edging down the touchline the instant he went through." "If anyone was going to be the victim, then this is the time, I reasoned.
But because the grass was a little dry and I would have ended up on my tummy, I was unable to perform the full Mourinho knee slide.
Moyes has earned a place in West Ham history with trophy-winning managers John Lyall and Ron Greenwood.
The club has now qualified for Europe three seasons in a row for the first time with a spot in the Europa League for the upcoming season.
He declared, "I truly believe the past two years have been a fantastic trip.
"It's a remarkable accomplishment to do so well in Europe, especially when you consider some of the extremely excellent teams that struggle. We've transformed a bad season into a brilliant season, which is fantastic.
