Question 


Prof. Frank Edwin intends to conduct a research into patients’level of satisfaction on the quality of health care at the Cardiothoracic Centre of the Ho Teaching Hospital. You are required to prepare a participants’ information sheet providing detailed information about the research. The information must include all the ethical principles that will be considered in conducting the research.

PATIENT'S LEVEL OF SATISFACTION ON THE QUALITY OF HEALTH CARE AT THE CARDIOTHORACIC CENTRE OF THE HO TEACHING HOSPITAL

Introduction

I would like to invite you to participate in a research project . Before you decide ,you need to understand why the research is being done and what it will involve for you . Please take time to read the following relevant information carefully and ask questions about anything you do not understand. Talk to others about the study if you wish or contact the reasearcher on the details provided,should you have any questions. Take time to read to decide whether or not you like to participate. Who am I and what the study is about ?


I am Daniel ofori Boateng and currently a level 100 student offering Medicine at the University of Health and Allied sciences. The main objective of this research,which would last for about a week , is to gather information on the level of patients satisfaction at the cardiothoracic centre of the Ho Teaching hospital to improve the quality of health care delivery and related services .


Why have you been invited



You have been invited to take part in this research as you are a stroke patient who is either attending or has attended the cardiothoracic centre at Ho Teaching hospital. Give your experiences of attending these sessions as your views, opinions, and perceptions are invaluable to the conduct of this study . The capacity of participants in this study is estimatedly 400.


Do you have to take part ?



Participation in this research project is entirely voluntary. Please note that consent to participate is  implied by you completing and submitting the questionnaire. Please indicate that you have read and understood the information provided here by ticking the box at the top of the questionnaire. You can withdraw at any time without giving a reason and without it affecting any benefits that you are entitled to.


What will happen to me if I take part?



You will be asked to complete either a printed or electronic (online) questionnaire. Please ensure you only complete one version of the questionnaire, which will have 16 questions. The questionnaire should take approximately 10 minutes to complete. The electronic questionnaire can be accessed on your charity social media site, such as Facebook. After submitting, the questionnaire will be forwarded to the researcher’s secure  account for analysis. The printed questionnaire with envelope can be collected from your charity. Once completed, please drop the sealed questionnaire into the box located at your charity reception area in order to fulfil your informed consent.The questionnaires will then be sent to the researcher at  University  of Health and Allied sciences for analysis.




What are the possible benefits and disadvantages of taking part in the reasearch ?




Whilst there are no immediate benefits for those people participating in the project, it is hoped that this work will enable patients receive the best of health care in the long run. It is possible that some questions could cause you distress, as they might remind you of potentially unpleasant experiences associated with attending the rehabilitation programme. Please seek support through your charity. . If you have a complaint about the conduct of this study, please contact Dr. Seth Kwadjo  Angmorterh  on  0556441769 to raise your concerns.

Will taking part be confidential 

All the information that we collect about you during the course of the research will be kept confidential. You will not  be identified in any ensuing reports or publications.All data is anonymous and no personal, or identifiable information is required as part of this study. Please ensure you do not disclose either your own or a third party’s personal information anywhere on the questionnaire. Data from electronic questionnaires will be stored on  University  of Health and Allied Sciences password encrypted database.



 What will happen to the results of the research study

The results will be used for the purpose of knowing how the duty of care with regards to patients , is adhered to . However, they might additionally be disseminated at conferences or published in peer-reviewed publications without disclosing any information about the participants. A report with the results will be sent to cardiothoracic charities and rehabilitation services. If you are interested in receiving a copy of the final report, please contact the  researcher  and it will be sent to you. 

Who is organising and finding the research 

This study is self-funded by the researcher Prof. Frank Edwin, a cardiothoracic surgeon at the Ho Teaching hospital and the Dean of the School of Medicine in  at University of Health and Allied sciences.


Who has reviewed the reasearch 



This study has been reviewed by the  Ghana Health Service in collaboration with the Ethical and Protocol Review Committee of the College of Health Sciences , University of Ghana, Korle Bu , Komfo Anokye Teaching hospital and the Ho Teaching Hospital. The reasearch being carried out in the Ho teaching hospital is adherent to all the existing protocols and principles underlying the conduct of reasearch.  You are entitled to expect the highest level of integrity from our researchers during the course of the research.
