It is the formal way of requesting for a system because all needed information about the system are documented and presented to an IS(Information System) body or manager. The purpose of System Request is to initiate or start a system or project.
WHICH AREAS AND INDIVIDUALS WILL NEED TO WRITE A SYSTEM REQUEST?
Individuals in an organization,firm or an institution will you need to be able to a write a system request to request for a system which they think will help improve the operation of their firm.
WRITING A SYSTEM REQUEST
Scenario:
You work in a private hospital and the system in place for recording patients records is book or paper system where records are recorded in a lot of books. You notice that this system brings a lot of problems such as delays in fetching patient records and even damage of records and therefore there is a need for the hospital to adopt a new system where patient records will be digitalized(Using websites,applications,computers).
First and Foremost, a system request is written to request for a system or project so the Name of the system/Project you are requesting should be the your Heading. From the scenario our heading will be
SYSTEM REQUEST – DIGITALIZATION OF PATIENT RECORDS.
The next thing will be person/s who brought the idea for the new system(Project Sponsors).Here the sponsor will be you yourself. Your name in full, department, the name of your organization,your phone number and email.

because from the scenario the paper system result in problems like delays in fetching patient records hence the need for an new system that is more efficient.

2. Digital technologies such as computers , servers and applications.

Business value are the values the business will gain when they adopt the new system. It is in two categories; Tangible and Intangible. Tangible value are values that can be measured,such as time and money and Intangible value are those that cannot be measured, such as improvement in health and reduction in stress.
