Forest trailed 2-1 after extra time in the second leg of the playoffs, making the score 3-3 on aggregate, but won 3-2 on penalties at the City Ground after three saves by goalkeeper Brice Samba.
The final save prompted a pitch invasion by home fans to celebrate the victory, with video showing one fan going up to Huddersfield captain Billy Sharp and headbutting him to the ground as he stood on the sidelines watching.
The event was dubbed "assault" by Huddersfield manager Paul Heckingbottom, who promised that the offender "would be dealt with."
Forest, a two-time European champion, last played in the Premier League in 1999.
